Make an observation traceable to its original record
A finding labeled Actual evaluator observation is still an assertion if the underlying session record cannot be found. Useful evidence needs a retrievable source.
Follow the record
Record the actual session date, anonymous session identifier and controlled location of the original notes. Preserve permission and restrictions separately. ConnectDent requires those references for actual observations and distinguishes them from its three sample examples. Do not select the actual class for a rehearsal.
Interpret the result
Another authorized reviewer should be able to retrieve the source and distinguish the participants statement from the teams interpretation. Saving a form or exporting a spreadsheet does not independently verify authenticity or establish customer adoption.
