Connect a clarification to the instruction it changes
A laboratory question can be answered in a message while the instruction used by staff remains unchanged. That leaves two conflicting places to look for the answer.
Follow the record
Inspect the material question in CD-1058. Save the response in the conversation and revise the instruction where its content changes. Explain why the revision was made. The question stays in the case history after the request returns for laboratory review.
Interpret the result
Ask another reviewer to reconstruct the issue, answer and current version. Count unresolved ambiguity rather than messages sent. The record should help both roles find the decision; it does not replace the responsible professional’s interpretation of the instruction.
